Community Health Fund

The criteria for bariatric healthcare professionals to apply for Formulite's Community Fund are as follows:

• Patient Eligibility: The patient must be booked in for bariatric surgery.


• Financial Need: The patient must demonstrate financial need and lack the means to pay for the supplements themselves.


• Application Process: Healthcare professionals (such as dietitians, surgeons, physicians or clinic staff) must apply on behalf of the patient.


• Confirmation: Formulite staff will call and email the rooms to confirm receipt of application and acceptance within 48 hours


• Funding Coverage: Formulite will cover 50% of the costs of Formulite's pre and post-surgery bundles for eligible patients by providing a unique discount code.
